Rule 5
Applying for Social Security Allowance

(1) In accordance with the Act and this regulation, a busy person who is eligible to receive social security allowance shall present himself at the ward office of the ward where he resides and submit an application in the format of Schedule-1.
(2) Notwithstanding anything written above, in the case of a child, his mother, father, guardian, person who takes care of him, and in the case of a person receiving social security allowance who cannot be present himself, his guardian, guardian or person who takes care of him can do so. 22 When making an application according to rules, two copies of the passport photo of the person receiving social security allowance should be attached along with the following documents:-
(a) Copy of certificate of citizenship in case of persons other than children,
(b) Copy of birth certificate in case of children,
(c) In the case of a disabled person, a copy of the disability identification card issued in accordance with the prevailing law, (d) In the case of the following single women, the following proof or document:- (1) In the case of a divorced woman who has not remarried, a copy of the divorce registration certificate and proof of no remarriage,
(2) Copy of husband's death registration certificate in case of widow,
(3) Evidence of judicial separation in the case of a woman who has been judicially separated, and
(4) Letter of recommendation from the local level in case of unmarried women.
(4) Notwithstanding anything written in sub-rule (3), if a copy of that sub-rule 800 cannot be taken down or submitted immediately, submit it at all.
(5) If the complete document is submitted according to sub-rule (4), a copy of the ward stamp should be taken down and certified and kept.
(6) The Hari Ward Office shall provide free of charge to the concerned person the application given under sub-rule (1). (7) The relevant ward office shall investigate and register the application made in violation of sub-rule (1) and above.
